As a Hospice Nurse, you would work with a team of healthcare providers to ensure our Hospice patients (prognosis of 6 months or less) are receiving the right care. Additionally, you would provide emotional support to the patients' families. This position offers potential for growth, a friendly staff, and an opportunity to interact with patients and their families on a more personal level. Please note: This position is on a Per Diem/PRN basis working as needed. This position will require travel throughout our service area in order to conduct visits at our patient??s home or nursing facilities. As a PRN RN, you may be asked to provide coverage during the evenings, overnights/on call and weekends. Primary Responsibilities: Conducting visits at home, skilled nursing, long term care and/or assisted living facilities Coordinating, assessing, and implementing patients' care Providing accurate and timely documentation of patient services to reflect plans of care Conducting admissions Interacting with other group members such as CNA's, LPN's, social workers and dieticians Performing physical activities that require considerable use of your arms and legs and moving your whole body, such as climbing, lifting, balancing, walking, stooping, and handling of biohazard materials Participating in an on call rotation ()
