VNA of California is looking for a RN Case Manager to assist with coordination of the program to ensure comprehensive quality care. Duties include: working with the overall coordination of the team to ensure that individual patients needs are met, acting as a liaison between the branch office, administration and all other departments providing information as needed, ensuring appropriate utilization and documentation of services for individual patients, and other assigned responsibilities to assist in coordinating the overall program. The ideal candidate will have a current CA Registered Nurse license, previous management or supervisory experience, and above average ability to use verbal, written and interactive communication skills.
