As a PT, you know that best results are often achieved when you work with patients in their home environment! This is a great opportunity for PTs who have at least one year of prior home health patient experience.In this important role, you will work directly with patients whose functional level has been adversely affected by a medical condition, providing initial assessments and interventions, as well as ongoing therapeutic services as indicated by the care plan. You will assist both patients and their caregivers in learning exercise protocols, including providing instruction, guidance, and follow up assessments. At VNA of California, our PTs work closely with patients to help them reach their maximum potential.To qualify for this opportunity, youmust be a graduate of an approved school/program of physical therapy, with at least one year of prior experience, preferably in a home health or rehabilitation setting.
